Can you try reverting back to the default PulseAudio configuration and see if
that helps?  If so, you can try turning your options back on to see what
triggers the failure.

In particular, I'm suspicious of these lines in daemon.conf:
> default-fragments = 2
> default-fragment-size-msec = 125

Does removing them help?
