kernel32 crash and backtrace

When attempting to open an Intercon Accessible FormNet VisualFill (v3.0.3.724)
[register to download at
http://www.gsa.gov/Portal/gsa/ep/contentView.do?bodyOnly=true&contentType=GS...]
form, got a kernel32 crash (see attached back trace).
