Starting executables with CMD START does not work.

Z:\home\phoe>start "" C:\Furcadia\Furcadia.exe
fixme:exec:SHELL_execute flags ignored: 0x00000100
Application could not be started, or no application associated with the
specified file.
ShellExecuteEx failed: Invalid parameter.

Reproduced on 1.9.21, 2.01 and 2.7.
