I can get the underun problems (and noise) using the demo from comment #66 (http://www.toymaker.info/Games/html/directsound.html) The patch is against wine-0.9.38. With this "hack", I can get it to play correctly (no noise/no real underrun - with or without hardware acc.) this dsound demo.
