if the error "Main_DirectDrawSurface_GetSurfaceDesc Impossible/Strange struct
size 0" is spit out many,many times you might want to run without debug
messages, and see if it speeds up the game a bit;
could you try WINEDEBUG=-all wine app.exe ?
