Debug log of unhandled page fault.

This is the log with WINEDEBUG set to

+mshtml,+shdocvw,+urlmon,+htmlhelp

To my untrained eye, the line immediately before the unhandled page fault looks
a bit suspicious:

trace:mshtml:nsChannel_AsyncOpen container = NULL
