After filling in all the fields for a new application version.  Pressing the
"Preview" button results in:

Warning: Cannot modify header information - headers already sent by (output
started at /home/winehq/opt/appdb/include/menu.php:35) in
/home/winehq/opt/appdb/include/util.php on line 10

and:

Oops            

Something went wrong with the application or version id

Blind submission of the information appears to work.
