[Bug 11099] custom checkbox not displayed

By using IE6, forms do display, though there are still issues. (see bug 32976)
With IE8 many error boxes pop up (Internet Explorer Script Error.) and the
forms then sometimes do not display. Try ie6 and see if that makes a
difference. 

TaxAct 2012 says that it requires IE6.
