[Bug 22572] Managed C++ assemblies don't work in Wine + Mono

So, my current plan is to work on getting Hello World working in Mono on
Windows, using a separate mscoree.dll. This should simplify things in Mono, and
make it possible for Wine's mscoree to work the same way.

For now, I'm trying to get some small cleanup into Mono (which they have mostly
ignored so far), and hoping Kornel Pal will help with the hard parts.
