Muhammad: This looks a lot like a PulseAudio bug. Your log continually prints
"state 3" which means the device is still "playing" even though we're not
providing it with any data. Can you report your PulseAudio and alsa-plugins
(called libasound2-plugins on Ubuntu) package versions?
