Oddly enough, once when I ran winetricks dotnet20, twisted's installer immediately popped up its gui from a previous run! Go figure. According to http://starship.python.net/crew/mhammond/win32/Downloads.html, the pywin32 installer is built using distutils (!). Sure enough, distutils seems to be able to build both msi and .exe installers.
