To allow participiants to get the trial version, I'd translate the form for You.

"Bezplatna registracia" means "Free (beer) registration"
"Meno" .. "Name"
"Priezvisko" .. "Surname"
"Heslo" .. "Password"
"Potvrdenie hesla" .. "Confirm password"

Then You can login and download the AMBULANCIA software.
