I can also provide a backtrace with r600g driver in use (if neccessary). If this bug has nothing to do with graphics stack please close Bug 22459. I doubt anyone has a copy of this 16-bit stuff anymore (dated 1994). @comment 3 - your backtrace is a different problem, completely unrelated to this bug (and most likely already fixed, see mmdevapi GUID).
