Alistair, do you have a joystick attached to your computer? I don't have a joystick and currently don't have access to one.
Maybe it's Debian specific.
I've just thought you have a special joystick because from your +dinput log:

However I'm still having the same issue. When I use the keyboard, the output appears in the terminal that I started the game in.
