I just tried again with a different dinput8.dll (from dll-files.com rather than
an old XP install), and this completely fixes the smoothness of the mouse input
for me.

It does introduce a new problem though - there now seems to be a box in which
the mouse can move which is smaller than the window, so you can't completely
move around the menu, and you can't look all the way around whilst playing for
instance.  The box does seem to move around if you click and drag though.
