After reboot it must be done again (the mounting part). If fuseiso in installed, it could be simplified to the user by allowing to point to an image file in winecfg, auto creating a drive_* folder and mounting an iso there via fuseiso on wine startup and unmounting on wine shutdown. Windows doesn't support this, why should wine?
Windows doesn't support alomost all settings present in winecfg, such as winecfg drives settings. Are there any difficulties with supporting image files? True, but that's because there's no way to do this outside of wine, which ISO handling can be done outside of.
