dmloader: Mark cached objects as loaded.

Otherwise, the object won't be found in the cache and will leak on
release.

dmloader: Don't use ReleaseObject() in loader_ClearCache().

Some objects have no GUID or name (e.g. segments created from MIDI files),
so ReleaseObject() won't find them in the cache.
