cmd: Fix some initial env variables setup.

- COMSPEC should be set to the path of running cmd.exe
  instance if not present,
- PROMPT must be set to $P$G if not already defined.

cmd: Ensure that all output to STD_OUTPUT go through WCMD_output_asis.

This allows:
- to also page formatted output (no reason it isn't),
- implement line buffering for STD_OUTPUT.
