Hello, I am Adeniyi Mayokun and I am interested in working with wine for
this year's version of GSOC. I need to know if there is a proposal format
to be followed and how can I contact my mentor

There's no set format. We can't tell you who would mentor without
knowing your project proposal. You can use wine-devel to contact
interested mentors.

Note that you must have gotten a patch committed to wine to be
eligible to participate
(https://wiki.winehq.org/Summer_Of_Code#Wine_and_the_Google_Summer_of_Code),
I don't see a patch from you in git yet..
