Quite easy. It's a simple text file filled with the exported functions like

DESCRIPTION  'My Special DLL'

EXPORTS
        CMRegisterControl                       @1
        CMUnregisterControl                     @2
        CMGetCommandString                      @3
        CMRegisterCommand                       @4

etc.

I'm sure you will find more info about this on the net.
