It has been reported that FPS slowdowns in VATS are only occurred with nVidia GeForce 7XXX or older, If you have nVidia GeForce 8XXX, 9XXX or even newer it should work better. It has been suggested that special shader replacements(smb30->smb20) for older cards could help as the new shaders seem to be mostly involved in vats and dialog sequences... I have this problem also when using Vista Business.. So I don't think this is a Wine related bug..

I recently tried the game under XP on this computer; the bug doesn't happen there.
It's definitely a WINE problem.
I confirm this. This solution works but I think it is not right. I know that the GL_ALPHA_TEST state is slow. I don't know another solution. I'm running Fallout 3 on a Nvidia 6800GT 128MB with lowest settings and it works :) - I applied the WinD3D patch as suggested on the appdb howto - commented two lines in stats.c - compiled - installed - retraced all installation steps from the playonlinux script http://www.playonlinux.com/repository/?script=402 However I didn't accomplish to build a deb file nor an improved playonlinux script. Hey Alexander. Please keep in mind while investigating this that, like other said, this issue exists in windows aswell so it could be an issue within the game.
I have tried to run this game on windows XP. This game is not faster in other scenes. I've succesfully applied this patch to wine 1.1.31 and i can say this patch solves the problem! I have prepared new patch. I have checked it with "GeForce 7000M" video-adapter on the "X80N" laptop. This video-adapter is very slowly for this game in all scenes. Please check this patch with more powerfull HW also. ( GeForce 7300 or better ).

The mentioned files has been changed since the 1.3.4 version, so patch for wine-1.2-rc4.f3 does not match. Also I have saw ano more bug: when you run in the V.A.T.S. the FPS slowdown with/without my patch ( thanks to Austin English. This bug realy is present. It seems to be caused by bug 30639 (http://bugs.winehq.org/show_bug.cgi?id=30639) and is not an issue in wine-staging. Note that this seems to have lowered the audio quality a fair bit, especially when it comes to the sound of lasers. The VATS slowdown, for me, is only experienced (again, in wine-staging) when I'm using the "emulate a virtual desktop" option. This is a pity because that option prevents issues with application switching and otherwise seems to have no detrimental affect on the game (apart from the VATS issue). I have also tried setting OffscreenRenderingMode to "backbuffer" which stopped the VATS slowdown but made a tonne of objects transparent. Pretty unplayable.
