  steps in Makefile.msc, with manual changes:
   - SQLITE_CDECL, SQLITE_APICALL, SQLITE_CALLBACK and SQLITE_SYSAPI defines;
   - remove wrong SQLITE_APICALL from varatgs sqlite3_str_appendf().
