	- Add handling for DDSCL_SETFOCUSWINDOW in SetCooperativeLevel.
	- Print a fixme for DDSCL_CREATEDEVICEWINDOW  and
	DDSCL_SETDEVICEWINDOW.
	- Don't allow DDSCL_EXCLUSIVE without DDSCL_FULLSCREEN.
