I had it happen in singleplayer.
I was playing The Passing, and the sound issues happened in the second chapter.
The game then broke when I finished the chapter. (Results screen flashed for
a second, and then I was taken back to my character's view, but unable to move
as the game prompted me to heal Rochelle.)
