Also, is there a preferably small particular application that exposes this problem?
I failed to reproduce the problem using the Quake 2 demo; I'm guessing that this is a D3D-only issue. I did manage to reproduce the problem (or at least I seem to) using the FIFA 2004 demo [1]. (A couple of seconds before it goes down in flames.)
