Debug log for SMTOI

In hopes of giving this "pet bug" a little more attention now that it's post
1.0 time, here's another log file, this time with +d3d9,+wined3d,+msgbox. It
was run on git version wine-1.1.1-191-g5798b95, the latest as of this writing. 
The symptoms are still the same as the ones noted above, although the numbers
are now (19, 84, 14).
