Upon opening the game, there are artifacts that follow the cursor. A temporary solution I found is to change the graphics mode from d3d/TNL within the game(it does not matter which order, just change them) and the graphics are normal one the screen refershes. The artifacts return upon exiting an re-opening the application. This is same with at least intel and radeon cards and different driver versions.

The artifact issue persists, but now additionally when the game starts, the loading screen remains forever. You can still click around and hear sounds, it seems the screen is not redrawn. Let me know exactly what's best to provide since the game is not crashing

Why are the bugs still open?
How are you getting this to work? Are you installing it or copying it over? Are you using a NoCD crack?
The GOG version is DRM-free and installs and starts without problems so long as directmusic is installed first. He may be referring to bug 13351, which is 6 years old but didn't have a known workaround until about a year ago; without that workaround, the game is unplayable.
As for artifacts following the cursor, I don't see any, but I have an Nvidia card with the proprietary driver.

I, with version 3.4 and 3.5 of wine, still have problems returning to the app's focus. Does not respond keyboard or mouse and also other cases in which artifacts appear (I leave screenshot).

As OP, a temporary fix that lasts until next restart, is to change display card settings. EE GOG Edition, Mac OS X 10.13.6 wine-4.21 (Staging).
I have seen someone mention how EE handles framebuffer versus how Wine does it, but I cannot seem to find that reference right now.

The output from starting EE GOG, vanilla wine-4.21 (Staging) on OS X 10.13.6, no winetricks nor configuration changes needed. Had to run from inside application folder.
Resulted in a screen as I posted before, attachment 65888, with cursor artifacts. After changing 3D Acceleration in Settings, swapping between Direct3D and Direct3D Hardware TnL, the artefacts are gone and screen looks good until next restart of the application.
At the moment, I used macOS Graphic drivers for my Nvidia GT 650M.
