I haven't customized anything in winecfg, except setting OS version to Windows XP. In Word, when I select Insert -> Picture -> Clip Art... , an error message appears saying "Clip Organizer cannot complete the operation. Error Code 0x80040155". In the wine log, I see:

And then Word freezes. If I kill it and then try to relaunch it, I get an error message saying Microsoft Word has encountered a problem and needs to close as it starts up. Nothing looks particularly wrong in the log as this is happening. At this point, the only way to get word to start again is to run winecfg and take out the overrides for ole32 and rpcrt4, but then I get the original error. This may have to do with the fact that I'm running WINE in Windows XP mode.
I just tried switching wine to Windows 98 mode, and without dcom98, I get the original error. With dcom98, Word tells me "Clip Organizer cannot complete the operation. Error Code 0x80004002. No errors appear in the Wine log though.

The problem is not actually with inserting clipart per se, it is trying to do it through the Clip Organizer. Selecting Insert=>Picture=>From File, browsing to the clipart directory, and selecting a file to insert, works fine. Selecting Insert=>Picture=>Clipart produces the error message mentioned in the original report. Clicking "OK" opens the Clip Organizer task pane, but it is nonfunctional.

Basically you will have to install MDAC 2.5 via winetricks.sh.
Afterwards search and insert functionality do work. Only the thumbnail does not rendered in the organization pane.
The whole office suite is impacted: Word 2007, PowerPoint 2007 and Excel 2007.

In Word 2003, trying to insert clipart now starts the installer, which tries to install the clip organizer (even though it is installed by default), then ends with a message that "installation ended prematurely because of an error." The clip organizer nonetheless opens, but cannot find the clipart--neither searching nor browsing works. The actual clipart files are installed just fine--I can see them in Konqueror, and even insert them into native Open Office. In addition, it is no longer possible to work around this by using Insert Picture=>From File, because it is no longer possible to insert pictures from a file at all. (Yes, I will open a new bug for that as soon as I run the regression test.)
For Office 2007, things are better, but still not entirely fixed. Searching clipart still does not work at all, but it is now possible to browse the collection through the clip organizer, select and copy an image to the clipboard, and manually paste it into the document, so long as you don't close the clip organizer before you paste it (otherwise it is erased from the clipboard, even if you tell Word to keep it available). Inserting an image by double-clicking does not work (it should).

In Office 2003, searches from the sidebar that opens when you select Insert, Clipart from the menu do not work. The full clip organizer can be opened, but it too does not work. (Cannot search, browse, or add clips to it.)
In Office 2007, searches from the sidebar also do not work. In the full clip organizer it is possible to browse, but not search, locally-stored files, and they can be dragged into the document but not inserted by double-clicking (as they can in Windows). Searching Microsoft's online clipart collection does not work.

The situation is the same as a year ago (comment 16) regarding the clip organizer and offline searches for both Office 2003 and 2007. Clicking "Search online" now opens Wine's IE to Microsoft's online clipart site, and it is searchable, but neither downloading nor copying images to the clipboard works.
