It seems that this is not an issue with Winecfg but is a problem in the code
that enumerates ALSA devices. If HKCU\Software\Wine\Alsa Driver\AutoScanCards
and AutoScanDevices are set to Y the cards are still not shown but there is an
entry "Intel ICH5" under Wave Out Devices instead of "dmixer". By forcing
DeviceCount to 2 and specifying DeviceCTL1-2 and DevicePCM1-2 it is possible to
get both cards displayed in Winecfg.

Summary: Winecfg does not show all ALSA devices
Product: Wine
Version: 0.9.6.
Platform: PC
OS/Version: Linux
Status: UNCONFIRMED
Severity: normal
Priority: P2
Component: wine-programs
AssignedTo: wine-bugs(a)winehq.org
ReportedBy: vesuri(a)jormas.com
I have two sound cards in the system, an integrated Intel ICH5 one and an
M-Audio Audiophile 2496 card. I've also created some slave devices for them
because the Audiophile, for instance, appears as a 10 channel device instead of
a 2 channel one when used directly. Winecfg, however, does not show these
devices. Under ALSA Driver I have the following:
ALSA Driver
[-] Wave Out Devices
| +-- dmixer
[-] Wave In Devices
| +-- Intel ICH5
[-] MIDI Out Devices
| +-- M Audio Audiophile 24/96 MIDI
[-] MIDI In Devices
+-- M Audio Audiophile 24/96 MIDI
I'd expect all the cards (and slave devices) appear as sound cards for Windows
programs. It's not obvious why only these devices are shown. There also is no
obvious way to change the ALSA output device if only one such device is
supported by Wine.

Well, i tested it already with a few earlier wineversions (bacck to oct. 2004)
but it keeps segfaulting here. Running in winedbg didn't work for me. So i dunno :)
For now i'll just add the download link
