Patch series to allow MS PDB server to work

Patch series does:
1. Adds support for context handles in stubless server code. This fixes the
following messages:
  fixme:rpc:calc_arg_size Unhandled type 30
  err:rpc:I_RpcReceive we got fault packet with status 0x3e6
2. Add support for [out]-only non-conformant strings. This fixes the following
message after the above is fixed:
  fixme:rpc:calc_arg_size Unhandled type 25
