All sound servers use sound devices in exclusive way. This prevents Wine from using any sound devices.
I can run simultaneously any number of audio clients and all of them are working normal, even more, I can run simultaneously multiple games under wine and they all have sound at the same time.
Remove all sound servers and let Wine talk directly to ALSA.
I wrote in my comment #1, I tried to use pusleaudio ONLY as last resort and normaly I use only ALSA!

Problem outside of Wine.
And you are incorrect, I have found the cause of problem, it's in the focus of window. If you start LotRO in an X session with no WM then LotRO think that it doesn't have a focus on it's window and disable all sounds other that sounds in movies and player conversations (LotRO is a first game where TeamSpeack 3 is tested), this behaver was tested on Windows XP (the real one) and if I Alt+Tab, no sound from game except of movies and player conversations.
And so it is clear that wine fails to correctly set active state for main window of LotRO.
