For non professionnal midi apps such as games, the first device is selected by default by the app which does not provide the ability to select another one which is particularily annoying, as in your case, when it is an external midi port. To address this, built-in synthetisers are enumerated first and then external midi ports. So there shouldn't be any problem. Could you enable midi debug channel with "WINEDEBUG=+midi wine ......" and add the log in the report so we can see what's going on ?
