Woohoo! I think I finally found the cause.

# xsetwacom --list
Wacom Intuos Pro M Finger touch     id: 13    type: TOUCH     
Wacom Intuos Pro M Pen stylus       id: 14    type: STYLUS

# xsetwacom --get 14 all

Hmmm, why does my stylus have a TapTime of 250?

# xsetwacom --set 14 TapTime 0

Now photoshop painting is fluid and pressure sensitivity works. The previously
mentioned brush-tip "spacing" option can use default settings now.
