(NEEDS WORK, rebased) Dsound resampler and cleanup patchset

Yeah, things are kind of in flux as they're being sent to Wine this week.
Here's another patchset. This should apply cleanly on top of
9c19ba63a84f0024e3bb0e4febc6da484b47d3e5.
