Fixed it. Removed the "art_cache_size=1048" line entirely from fallout2.cfg, as recommended on another forum, and now the game loads.
If you have Windows installed, you should check if the game starts correctly with that setting. If the game crashes on Windows as well, this bug report should be considered invalid. The relevant line in the gog.com version of the game is "art_cache_size=8" -> the game starts normally here. If I change the value to "1048", it indeed crashes.

I'm able to test in Windows too. If windows works with the standard game values I think the bug is valid.

I also tested in Windows XP (art_cache_size=1048) and the game is playable too.
So unfortunately I'm not able to see the original issue.

When I load the game, the please stand by screen appears and then dissapears. Nothing happens after, I have to kill the game manually. I don't remember having a problem like that when I tested the game for this bug report a couple of years ago. Can somebody else test this and tell me if they get similar results to me? Thanks.

I have installed the GOG version of Fallout 1 on wine-1.7.45 and what you described occured. I was able to fix it by creating a DLL override for ddraw as described in a Fallout 2 test result (https://appdb.winehq.org/objectManager.php?sClass=version&iId=27466).

No, please read above, particularly comment 17. The GOG version has a workaround, different to the original. It is not a sufficient test.

Fallout 2 1.02.25 (Not GOG), Interplay branded installer for Fallout Trilogy, referencing Gamespy Arcade (defunct), defaults to "c:\Program Files\Interplay\Fallout 2 1.02.25", has no art_cache_size set in fallout2.cfg before running. After running, art_cache_size=8 is present.
Setting it to 1048 causes it to prematurely exit at start. Running under winedbg does not catch any crash.
Windows 10 under VirtualBox:
Same installer, runs fine with both art_cache_size=8 and art_cache_size=1048. It does not prematurely exit.
