I normally run all games in additional X session (using "startx lotro.sh -- :1 -br -once" command), this is much "safer" for desktop settings and work I do currently also much more reliable switching, but with LotRO there is no sound in it, if it started like that, if I start LotRO in my current desktop, there is a sound in LotRO. I also can make LotRO to work with sound in additional X session if I start there any "Window Manager" (twm for example) along side with LotRO.

What sound system is select in winecfg?
I tried with alsa and pulse audio(system wide and per-user setups), also used pulse-oss start up with LD_PREFIX(analog of using padsp). In all of the noted config's alsa was selected in winecfg, except when pulse-oss was used oss of course.

I didn't got the question, if you asking if any of noted sound server is running at the time I start the game then yes, they are running, and I can say that if I start any WM there is starting a sound to come from game and if I kill that WM there is again no sound.
And also I have noted that when I use alsa there is a sound at intro movie but if I use pulse audio (I use it through /etc/asound.conf by using pcm.!default and ctl.!default keys to forward sound to pulse) there is no sound at all (other then if I start and WM).

All sound servers use sound devices in exclusive way. This prevents Wine from using any sound devices.
I can run simultaneously any number of audio clients and all of them are working normal, even more, I can run simultaneously multiple games under wine and they all have sound at the same time.
Remove all sound servers and let Wine talk directly to ALSA.
I wrote in my comment #1, I tried to use pusleaudio ONLY as last resort and normaly I use only ALSA!

And you are incorrect, I have found the cause of problem, it's in the focus of window. If you start LotRO in an X session with no WM then LotRO think that it doesn't have a focus on it's window and disable all sounds other that sounds in movies and player conversations (LotRO is a first game where TeamSpeack 3 is tested), this behaver was tested on Windows XP (the real one) and if I Alt+Tab, no sound from game except of movies and player conversations.
And so it is clear that wine fails to correctly set active state for main window of LotRO.
