Attached is a log that I was able to get before the crash. It seems strange, but it appears from the number of release and cleanup calls (I deleted most of them, they just seemed to repeat) that something is causing Counter-Strike to exit (note that it hasn't crashed at this point,) cleaning up after itself. It crashes after this point with the stuff I've mentioned earlier, so I focused on the calls right before the Release calls in the hope that they might be useful.
