The German version of Baldur's Gate (v. 1.1.4315, the latest official patch, plus the DirectX 8 patch) hangs at the Intro screen when any of the available Buttons are clicked, be it "Spielen" (Play), "Konfig" or "Beenden" (Exit) etc. Sound plays at first but stops as soon as any button is clicked. The only terminal output printed by wine is
err:ole:CoInitializeEx Attempt to change threading model of this apartment from multi-threaded to apartment threaded
The Wine version is the wine-1.5.3-155.1.x86_64 RPM package for SuSE 12.1.

I can only guess what is happening on your system. One easy way to accomplish stopping/starting PA is with pasuspender, run: pasuspender -- wine "blah.exe" opt1 opt1...
Note: I'm trying to rally to have the winealsa driver do this on it's own when desired.

Using winecfg, what does the Audio page say for "Selected driver" AND the settings for output and voice output device?
If you wish to alter this the setting is in regedit here: HKEY_CURRENT_USER\Software\Wine\drivers
Jerome Leclanche, Please don't assume that any/every system that has PA installed is suffering from the same issue, there are now a number of moving parts and any of them can have an issue. We need to collect a base line set of information in order to classify bugs like this.
