I tried many versions since 0.9.49, but it looks like MT4 never worked correctly.

Hello Focht, reopening. You are probably using a diferent version (compare comment #0 with your screenshot - for example the "New" icon with green plus sign). Reported MT Version v4.00.225, build date 2009-07-10.
Still present in Wine 1.7.28.

Around Wine 10.2 (workaround Bug 57950) the icons look good.
If you have Wine Stable (10.0) installed, can you retry with that?
