Running openSUSE 12.1 x86_64, Wine 1.5.3 from the repositories. Followed all the instructions over here: http://wiki.winehq.org/MIDI . Midi playback works with aplaymidi, I created all the recommended registry entries and tested the MIDI playback in Wine with the MCI shell, that was successful.
When I try to run Little Big Adventure for Windows, i.e., the official Windows port of the original DOS game, (http://www.magicball.net/downloads/programs/lbawin ) I get CD music, sound effects and voices working but there's no midi music playback. I attached the log file output, the following error is shown:

Do you have real MIDI HW attached to your serial port? Your log shows no mention of SW synthesizers. Alas, the log doesn't tell why the app doesn't even try to play MIDI. Are you sure that it should play MIDI music?
Sadly, I'm not familiar with the fixme about the mixer and can't tell whether it matters.

You don't get CD music from that log with your drive F. Please read bug #20555 about subtle differences in the configuration. It also contains examples of using the MCI shell to play CD audio.
Something else is bogus about your log. It shows no trace of +driver,+tid
