Under 1.3.19 Fallout 3 immediately hangs. Switching to another TTY works, and when I switch back to the GUI all I see is black (from the application being in fullscreen mode). I can Alt+Tab away from Fallout 3, though once Alt is released the black screen returns (however keyboard input will affect the window selected using Alt+Tab, you just can't see it). Killing the process fixes everything again.
Wine also doesn't return anything to the terminal, when usually it returns a load of fixmes. When I terminate Fallout3.exe I just get 'Segmentation fault'.
If I downgrade to 1.3.18 it works fine.

Do you get console output with, say, WINEDEBUG=+file,+seh ?
I'm running Arch Linux and got my wine here: http://svn.archlinux.org/packages/multilib/x86_64/wine/. This is an Nvidia Fermi chip, 570GTX card running with the Nvidia driver 270.41.06.
I'm not sure what was going on with the console output yesterday, I tried again just now and I'm getting something from it.
The hang seems to happen before output regarding XLive is echoed - I've attached before and after outputs with .18 and .19. Output with WINEDEBUG set as you described is massive - I will attach if it's useful, but I'm guessing that was to get some output going.

I will if that's what's needed, but it might be a few days before I get my next quota of messing-around-with-wine-time.
