server: Fix the handling of the signaled status for file descriptors.
File handles are signaled when an I/O operation completes, and reset when another operation is queued.

As for the regression test, I had to mark 0.9.33 as good, and 0.9.35 as bad. Bisecting in the middle of 34 and 35 won't work for some reason. Moving one release past that seems to work okay (not sure what would happen if in the new bisect, the 'middle' patch falls in the same place...I'd presume it'd fail).
