Occurred with Dragon Age Origins 1.04. Reproducible with Wine 1.4.1 and 1.5.9.
With the first start, everything is okay. After exiting and restarting, the keyboard is dead. You need to reboot the whole system.

Do you have anything else installed in the same game prefix? Try wineserver -k for the game prefix before launching it again and see if that helps.

wine executable terminates while the game is still running. When I exit the game, I do not see any obviously wine-related tasks in the process list remaining. I have no dll overrides.
