On Thunderbird's advice I tried readtex mode a while ago, and got what you
describe: in game menu not drawn, but input still works and you get back to
game.

The hang truly is a hang:  freezes X for a time, before I can bring up a
terminal and kill it.  Never responds to input again, and no sound/video.

I'll plug the old card back in and get you a screenshot.

I have 256mb AGP aperture size set too, but I meant the card's onboard memory: 
doesn't wined3d make an assumption somewhere like 32/64mb?
