As of today with the new speaker configuration when a device is selected it
will not enable the apply button, I have to change any other config for the
button to enable.

I have 3 options for devices:
Out: HDA Intel - MID ALC272X Analog
Out: default
Out: HDA Intel - MID HDMI 0

Choosing any of them will not enable the Apply button.

Tested in an existing and new wine prefix. Besides that the selected
configurations do not seem to be saved because when I reopen the original
selections are there.

Works for me - I think you're misunderstanding the interface. "Apply" won't be
enabled when you change the device box because you're not actually changing
anything until you change the speakers combobox.

You are right, now I understand that the config is different for each device.

Maybe there should be a <select one> or something like that as the standard
selected item and the speakers would be disabled until a real device would be
selected.
