wined3d: Hide WINED3DFMT_R16G16B16A16_UNORM again.
This format is broken on some cards. Hide it until we figure out a reliable
way to deal with it.

wined3d: Rename some GLSL instruction handlers.
The pshader_glsl_* names are a leftover from when we had separate code for
vertex and fragment shaders.
