The accepted way is to include a "From: " line in the body of your email message stating the original author of the patch.
Do you mean just a "From: " line, or (as Alexandre requested for forwarded patches some time ago) the complete output of git-format-patch (this includes a "From: " line and a "From " line, the latter containing the commit object id?
