Whoops - I guess Wine does have the same system-level mutex.  :)  I just 
read through the syslevel documentation on WineHQ.

Since the DirectDraw stuff doesn't seem use the Wine syslevel (from a 
cursory examination), I suppose the flag is still a noop.
