+       VariantClear(&d);
+       V_VT(&v) = VT_BSTR;
+       V_BSTR(&v) = SysAllocStringLen(NULL, 0);

This VariantClear call attempts to free uninitialized VARIANT instance.
You probably meant VariantInit().
