I think this is the root of the problem:
Log: Opened viewport Log: Enter SetRes: 512x384 Fullscreen 1 Init: No acceptable display modes found (D3D_OK) Critical: Error setting display mode: No acceptable display modes found (D3D_OK). Please delete your UT2003.ini file.
In the part where it was enumerating the modes, I did not see a 512x384 mode go by. Try renaming that file and see if it helps.
